Under this model, primary care doctors manage all of the patient’s care and coordinate with specialists to improve quality and reduce costs.
In many other countries, doctors are routinely available after hours and on the weekends. Not so in the U.S., but that may be changing.
A “medical home” aims to be a practice where the doctor doesn’t keep you waiting, does keep you healthy, and works with a team to deliver better care cheaper.
Despite the anecdotal evidence, substantial data has yet to emerge showing that the medical home model saves money or lives.
Swedish Community Health is a pilot program, built on a model called the patient-centered medical home. The focus is on wellness, disease prevention and day-to-day management of chronic illnesses.
