State disciplines health care providers
Periodically Washington State Department of Health issues an update on disciplinary actions taken against health care providers, including suspensions and revocations of licenses, certifications, or registrations of providers in the state.
The department has also suspends the credentials of people who have been prohibited from practicing in other states.

The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998.

Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Note: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
State disciplines health care providers
OLYMPIA - The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Clark County
In June 2012 the Nursing Assistant Program charged certified nursing assistant Jennifer Dawn McCann (NC10017642) with unprofessional conduct. McCann allegedly diverted controlled substances from more than three patients for her own use. The Department of Social and Health Services issued a finding that McCann neglected and financially exploited a vulnerable adult.
Cowlitz County
In June 2012 the Health Care Assistant Program charged Virginia M. Knight (HC60211149) with unprofessional conduct. It is alleged that Knight tried to use a patient’s urine sample in place of her own during a workplace drug screen, admitted to using marijuana and taking unprescribed Xanax.
King County
In June 2012 the Pharmacy Board charged pharmacy technician Mesfin A. Gedamu (VA60067079, VB00070803) with unprofessional conduct. Gedamu allegedly diverted a controlled substance from the pharmacy where he worked, gave it to a woman without a prescription, and failed to respond to a pharmacy investigator about the conduct.
In June 2012 the Midwife Program charged Pamela A. Golliet (MW00000296) with unprofessional conduct. Golliet allegedly provided care that fell below the standard for a practicing midwife in Washington while delivering a baby, treating the mother, and during post-partum care for both.
In June 2012 the Nursing Assistant Program charged registered nursing assistant Mekdes Haile(NA00170503) with unprofessional conduct. Haile allegedly failed to assess a patient for injury after a fall before lifting the patient, which is a required policy of the assisted living facility where she worked.
In June 2012 the Surgical Technologist Program charged Jlynn Reeves (ST00002295), also known as Chris Reeves, with unprofessional conduct. Reeves allegedly filled unauthorized prescriptions for a controlled substance for himself and his 15-year-old son. He also allegedly failed to comply with terms of his substance abuse monitoring program.
In June 2012 the East Asian Medicine Practitioner Program charged Laura Marie Zanieski(AC00002226) with unprofessional conduct for practicing and billing for acupuncture sessions while her license was expired.
Kitsap County
In June 2012 the Nursing Commission charged registered nurse Tarra D. Lawson (RN00137654) with unprofessional conduct. Lawson was convicted in 2011 of second-degree organized retail theft, possession of — methamphetamine, delivery of a controlled substance, possession of marijuana with intent to manufacture or deliver, and second-degree unlawful possession of a firearm.
In June 2012 the Chemical Dependency Professional Program ended the probation order against chemical dependency professional Krissy M. Remeto (CO60151888), also known as Krissy M. Murphy.
Mason County
In May 2012 the Nursing Assistant Program granted the certification of nursing assistant Ashley Kristina Rose Carr (NC60250737). Carr was convicted of reckless driving in 2007, and admitted that she had abused drugs and alcohol.
Okanogan County
In June 2012 the X-ray Technician and Radiologic Technologist Programs charged X-ray technician and radiologic technologist Travis J. McFarland (XT00006387, RT60056326) with unprofessional conduct. McFarland allegedly forged a prescription for a controlled substance, and tried to fill it at a pharmacy. He also failed to comply with terms of his substance abuse monitoring program.
Pend Oreille County
In June 2012 the Nursing Commission charged registered nurse Rhona J. Hofman (RN00115099) with unprofessional conduct. It is alleged that Hofman practiced as a registered nurse for about 14 months while her license was expired.
Pierce County
In June 2012 the Nursing Assistant Program charged registered nursing assistant Peter Gichimu Kimani (NA60167029) with unprofessional conduct. Kimani allegedly neglected a vulnerable adult and has been restricted by the Department of Social and Health Services from working in a position that involves unsupervised access to vulnerable adults.
In June 2012 the Nursing Assistant Program charged registered and certified nursing assistantLynniah Grayson (NA60226177, NC60239688) with unprofessional conduct. Grayson was convicted of third-degree assault and first-degree theft in 2012.
In June 2012 the Nursing Assistant Program charged registered nursing assistant Tina Anne Grove(NA60230681) with unprofessional conduct. Grove allegedly provided care that fell below the standard for a nursing assistant leading to a patient’s death.
In June 2012 the Agency Affiliated Counselor and Nursing Assistant Programs charged agency affiliated counselor and registered nursing assistant April A. Stewart (CG60187202, NA00191503) with unprofessional conduct. Stewart allegedly tested positive for methamphetamine and marijuana during a urinalysis, and neglected a vulnerable adult who was living with her.
Skagit County
In June 2012 the Dental Commission amended a statement of charges against dentist Sam Oh(DE00010691). Oh was convicted of one count of criminal solicitation to commit a violation of the Uniform Controlled Substances Act — possession of Hydrocodone, and one count of criminal solicitation to commit a violation of the Uniform Controlled Substances Act — possession of Alprazolam.
Spokane County
In June 2012 the Chiropractic Commission charged chiropractor Chad W. Bess (CH00034347) with unprofessional conduct for failing to comply with a previous stipulation.
In June 2012 the Nursing Assistant Program charged certified nursing assistant Terra L. Callaghan(NC10058912) with unprofessional conduct. Callaghan allegedly took a resident’s check without permission and her sister tried to cash it.
In June 2012 the Massage Program granted the credential of Jaclyn Michiko Tsukamoto(MA60277366) and placed it on probation for at least three years. In 2002, she was convicted of two counts of federal misprision of a felony; in 2005, conspiracy to deliver a controlled substance — cocaine, and two counts of delivery of a controlled substance — cocaine.
Thurston County
In June 2012 the Agency Affiliated Counselor Program charged agency affiliated counselor Lauren Frances Robbins (CG60173525) with using more than $500 from patient accounts to buy controlled substances.
Whatcom County
In June 2012 the Chemical Dependency Professional Program ended the probation order against certified chemical dependency professional trainee and chemical dependency professional Tiffany Shireen Barley (CO60142735, CP60183406).
In June 2012 the Health Care Assistant Program ended the probation order against Eudelia Marie Starr (HC60134606).
